Facility Request Form

Facility requests are made utilizing the "Facility Request Form," provided below. Students should submit completed forms to the Office of Student Activities for approval. Faculty and staff should submit completed forms to the respective Vice President for approval, and external groups should submit completed forms to Brandie Keesee (bkeesee@atlm.edu, 404-756-4442).
